The social worker would be responsible for providing case management services for inmates, who often have a variety of complex needs such as insurance, court and legal appearances, homelessness, child custody, employment, and financial planning.
The social worker will work with inmates to ensure they can adequately prepare for and access community services upon their release from the facility.
In addition to this, the social worker will make referrals to other team members to ensure that inmates' behavioral and mental health needs (e.
g.
, medications, therapy) are being met while they are incarcerated at this facility.
Healthcare delivery in a correctional setting is challenging and rewarding.

We are looking for a highly motivated, empathic, and organized individual to join our behavioral health team.
This is a position funded through opioid abatement funds for one year with a strong likelihood of recurrent funding.
Essential Functions Creating case management plans which address the inmates' resource needs during their incarceration and providing referrals to other members of the behavioral health team as necessary (e.
g.
, therapy services, psychotropic medications) Developing plans and working with inmates to connect them with available resources for when they are released from the facility (e.
g.
, housing, community treatment programming, employee assistance, child custody coordination) Collaborating and building relationships with staff at community resource sites Communicating with other behavioral health and medical staff about case management and therapeutic needs, including during treatment team meetings Participating in team meetings with the treatment staff, on a weekly basis Carries out existing policies and procedures Requirements At least one year of relevant experience Ability to effectively communicate with other behavioral health providers and make referrals to other as needed Ability to build strong relationships with individuals who are incarcerated at this facility as well as staff at community resource sites Knowledge of available community resources and ability to assess inmates' needs upon release Ability to deal calmly and rationally with irate individuals Ability to establish priorities and make correct decisions based on valuable information Education Requirements Graduation from an accredited educational institution with a degree of Bachelor of Social Work (BSW) Preferred qualifications Prior experience working with underserved and vulnerable populations (e.
g.
, people with substance use disorder, individuals who have been involved in the legal system) Prior experience working in a detention facility Experience working within interdisciplinary teams and integrated care settings Pay Scale, Work Hours and Benefits Pay:
